VSC aims to support the management of complex patients in the community, strengthen continuity of care, and ease pressure on outpatient departments across Victoria.

Exclusion Criteria - Oxygen needs of 6L or more at rest or awaiting oxygen assessment (will accept on bridging oxygen)
