Purpose: A virtual televideo service allowing the MED 5B team to book patients in for a reviewe by a General Medicine physician within a week of their Northern Hospital discharge. This appointment is organised by the discharging medical team.

Suitable reviews include:

  • Assistance with follow-up of renal function/LFTs post-medication titration (i.e. withholding/ restarting anti-HT)
  • Adjusting antibiotic duration following response
  • ‘Screens’ – osteoporosis, myeloma, pre-biologics, etc.
  • Investigations not yet reported  (e.g. TTE, RFTs, NCS)
  • Follow up of scans with results pending
  • Assistance with prednisolone weaning
  • “High-risk” discharges (e.g. patient-initiated discharges)
  • Titration of analgesics
  • Titration of hypoglycaemic agents (SGLT2s in particular)
  • Recommencing statins
  • Other things where historically you have written “GP to chase” in the discharge are likely to be suitable
  • Specialist palliative care with a life-limiting condition
  • Preoperative education
  • Postoperative advice/management
